Online Patient Registration Form

Patients should arrive in our office 15 minutes before their scheduled appointments to provide new patient information or update their records and insurance information.

We pride ourselves on being on time and receiving our patients in a timely manner. That's because we value your time and do our very best to stay on schedule, but we can't do that alone. Please help us serve you best by arriving early to complete updates to your records, or by giving us at least 48 hours notice that you need to cancel or reschedule your appointment.

Financial Arrangements and Payment Policies
We accept most dental insurance plans, and accept assignment of benefits as a courtesy to our patients. Please check with our front office staff to confirm that we accept your plan prior to your arrival. Patient co-payments are due at the time services are rendered. Payment is expected when services are performed. For your convenience, we accept cash, check, Care Credit, and major credit cards.

After Hours Answering Service
Someone is available 24 hours a day to speak with you about your dental emergency. Please be aware that this number is for our current patients only, and cannot be used to schedule or cancel an appointment.
